HeartBrother Volunteer Jay Toland Named Patriots Difference Maker of the Week

We are thrilled to share that HeartBrother volunteer Jay Toland has been recognized as a Patriots Difference Maker of the Week—an incredible honor celebrating individuals who go above and beyond to serve others in their community.

The Patriots Difference Maker of the Week award is part of the Patriots’ Celebrate Volunteerism initiative—an effort by the Kraft family, the Patriots Foundation, and Gillette to highlight dedicated volunteers, raise awareness of community needs, and inspire others to take action. Throughout the 2025 season, the Patriots are shining a light on volunteers across New England who support children, families, and those facing life’s toughest challenges.

Jay embodies the spirit of this initiative through his unwavering commitment to the HeartBrothers Foundation. As a longtime volunteer and heart transplant recipient himself, Jay dedicates his time, energy, and heart to supporting others on the same journey. Whether he’s comforting patients and families at a hospital, mentoring newly diagnosed individuals, or helping raise funds by volunteering at a fundraiser, Jay’s compassion leaves a lasting mark on everyone he meets.

In honor of Jay’s incredible volunteer work, the Patriots Foundation is awarding the HeartBrothers Foundation a $10,000 grant—funds that will directly support heart failure patients and their families through housing, education, and community programs.

Jay will also be honored on the field during the January Patriots vs. Dolphins game, where he’ll be recognized for his extraordinary volunteerism and contributions to the community.

Through this recognition, Jay joins a remarkable group of New Englanders whose volunteerism is making a real difference in the lives of others. His story is a reminder that even one person’s commitment to kindness and service can inspire countless others to do the same.
