From Survivor to Supporter: Jay Toland’s Impact

When it comes to bringing hope to patients in the darkest of times, few embody the HeartBrothers mission like Jay Toland. A heart transplant survivor himself, Jay has turned his own journey into a lifeline for others. Since joining Team HeartBrothers, Jay has visited more than 500 patients across hospitals and transplant centers, offering not just encouragement but real-life proof that there is life after transplant.
Jay understands the fears and questions patients face because he’s lived them. That shared experience allows him to connect in a way that few others can. Whether it’s sitting quietly at a bedside, answering tough questions, or simply being a steady presence, Jay shows patients and families that they are not alone.
His impact goes beyond numbers. Each visit is a story of comfort, reassurance, and hope—and a reminder that the HeartBrothers community is walking with patients every step of the way.
